Three years after the atomic bombing of Hiroshima, adolescent Gen Nakaoka lives in a makeshift wooden shack with his ailing mother Kimie and his adopted younger brother Ryuta. Despite extreme poverty, social ostracization, and the ongoing trauma of radiation sickness, Gen maintains an indomitable, joyful spirit, selling scrap metal and shoe shines to buy medicine. Gen and Ryuta befriend a band of hardened war orphans living in the ruins, led by a disfigured boy named Katsuji. Together, the children navigate exploitation by corrupt black marketeers and American occupying soldiers while striving to rebuild their lives.

Gen works tirelessly to nurse his beloved mother, but Kimie's radiation poisoning enters its terminal stage, and she passes away peacefully in their humble shack. Shattered by grief, Gen almost loses his will to fight, but the memory of his deceased father and mother's unwavering courage restores his determination. Gen rallies the street orphans, helping Katsuji overcome self-loathing regarding his burn scars through artistic painting. The children pool their scrap metal earnings to construct a modest shared home, standing together on a Hiroshima hilltop as wheat sprouts through the ashes, symbolizing hope and resilience.
Barefoot Gen 2 struggles to reach the same emotional heights of the first film but it remains an engaging experience that informs and educates about life after the bomb.
